Gleeson College views the education and care of our students as a partnership between the students, parents/caregivers and staff. Communication with parents is of paramount importance to us.

Parent involvement in the committees of the Parents & Friends Association, the College Board, or in a voluntary capacity, is always highly valued and appreciated.

Medical Forms


Please find below some useful forms available for families to download.

Completed forms should be sent to our Senior First Aid Administrator, firstaid@gleeson.catholic.edu.au for both current and future students.

Medical Information Form

For all new and current Gleeson College students.  Students with health conditions must provide additional documentation / medical agreements.

Individual First Aid Plan

Required for children and young people when the first aid response is not the standard first aid response for that health condition.

Non Specific Health Care Plan

Must be completed for children and young people requiring additional care or supervision related to their physical or mental health and wellbeing where there is not a specific health care plan in place.

Medical Agreement

Medication agreements (previously medication authority) are required for all medications requiring administration in education and care services. This includes all prescribed and over the counter medicines.

Anaphylaxis

The following Action Plans must be completed by the student's doctor.
These forms are available from your GP or family medical centre.

  • Action plan for students at risk of anaphylaxis. For use with EpiPen.
  • Action plan for students at risk of anaphylaxis. For use with adrenaline (epinephrine) autoinjectors.
  • Action plan for students who may suffer from allergic reactions.

Asthma

Asthma Care Plan.
To be completed by the student's doctor and parent/guardian. These forms are available from your GP or family medical centre.
